Avoid CT of the sinuses and unnecessary antibiotic prescriptions for uncomplicated acute rhinosinusitis

22. JULI 2025

Rationale:

Viral infections cause the majority of acute rhinosinusitis cases, and fewer than 2% progress to bacterial infections. Most cases resolve without treatment within two weeks. Uncomplicated acute rhinosinusitis is often diagnosed clinically, and nasopharyngeal culture, CT of the sinuses, or other imaging is not indicated. Antibiotics are not recommended for patients with uncomplicated acute rhinosinusitis who have a mild disease course and access to follow-up care. If antibiotics are deemed necessary, phenoxymethylpenicillin is the first-line choice for most patients.
